Hello! This is our somewhat “secret” page with relevant forms.

FOR FAMILIES WHO ARE SEEKING CARE, PLEASE FILL OUT THE SIMPLE 2-PAGE APPLICATION. Email it to Meredith (admin [at] sunflowerpreschoolpdx [dot] com).

  • Fill out the 2-page application. This helps us gather your contact info and add you to our spreadsheet.
  • Write an application fee check (see /tuition page for amount) made out to Sunflower Preschool LLC. Drop it off at Sunflower Preschool (5705 SE Knapp)
  • Come take a tour if you have not already done so.

FOR ENROLLEES HEADING OUR WAY (hurrah!) HERE IS WHAT YOU NEED TO DO.

  • Here’s a 1-page overview of the enrollment process (contact info for Sunflower and Mud Pies, summary of forms, etc).
  • Here are links to the forms:
    • Mandatory
      • Child Care Enrollment (PR-0185)(This is a licensing requirement.)
      • Contract (This is a business requirement.)
      • Nutrition First Enrollment Form (This is the form we need for the USDA food reimbursement program.)
      • Current Vaccination Record of Child (or vaccine education module certificate)(We are required to report our vaccination rates to the county every year.)
    • Other
    • Email the relevant forms above to Meredith: admin [at] sunflowerpreschoolpdx [dot] com)).
  • For payment, please:
    • write an enrollment fee check (see /tuition page for amount) to “Sunflower Preschool LLC”
    • drop it off at Sunflower Preschool (5705 SE Knapp)
    • text Meredith (908-601-3905) when you drop off the check so she can be sure to check the mailbox sooner rather than later (she checks snail mail even less frequently than email)

ALSO, PLEASE NOTE:

  • Here is our Parent Handbook (pdf last updated June 2026). We realized it was weird to issue the Parent Handbook *after* enrollment, so now we make it available beforehand.
  • Here is the List of Things You Need to Bring on the First Day of School:
    • a labeled water bottle (that lives at school)
    • a small blanket (for rest time)
    • a lovie/stuffy (to go with the blanket, for nap time, if your child likes lovies/stuffies)
    • any diaper cream if needed
      • and an authorization form said diaper cream (see above link for form)
